When I heard it in Azumanga Daioh for Osaka, it kind of made sense. Being from Osaka was her gimmick. I don't think there's ever a time in Mai-HiME where any of the characters or even Shizuru herself mention the kyoto-ben. No "your accent sounds...funny" moments or anything. So I guess Bandai Entertainment just tossed in what they felt worked. Melanie Risdon's REAL voice can be heard as Youko (yes, she voices Youko too, I am dead serious).

Osaka and Kyoto are in KANSAI.
So that's why people from there speak almost in the same way.
And it's not really an accent, like saying that you talk with a British accent or an Australian accent or whatever.
It's more with the way they use the japanese, the modisms and so on.
A clear example is:
Ookini - which we all know it's Thank you.
But in Tokyo and that area (which is Kanto); you don't use ookini. You use the common Doumo or Arigatou.

This also happens in Final Fantasy XIII. Fang and Vanille have an Australian accent in the american version. But in the japanese version they just use a very informal way of japanese. So far I know, they don't use kansai-ben or any other ben.

But for dubbing purposes, they have to distinguish it somehow.
